To be fair, that second one is probably for movers or for getting heavy equipment or furniture around. Not all ramps are for wheelchairs. It's also common in many countries to have narrow ramps beside outdoor stairs for pushing bikes up or down.
Still, there are tons of wheelchair ramp fails out there. Here, have some more:

I have used one of those rail ramps inside hotels before.They are stable enough with a manual chair,but wouldn't want to use it with a heavy power chair.
